Student elections begin at 8am april 1 and end at 5pm april 2.

Election Forms

The following forms must be completed by regarding elected office at Illinois State University. Unless otherwise specified, all forms listed below are to be filed with the Student Elections Committee by submitting the form to the Student Elections Manager.

NOTE: These documents cannot be submitted electronically.  All forms linked from this page must be printed prior to submission.    

Candidate Platform Statement for Online Ballot

During online student elections, student voters are able to click on links to view short platform statements from each candidate.  In order for candidates' statement to be viewable online, they must submit the Candidate Platform Statement for Online Ballot form with their Petitions for Candidacy.

Candidate Registration Form

Candidate Registration Forms are not available online.  Candidates must complete this form at the Candidate Information Sessions in order to receive a Petition for Candidacy. 

Financial Disclosure Form

Each student candidate or political organizations must keep accurate and up-to-date records of all campaign receipts and expenditures. Receipts must be provided for all campaign expenditures. All expenses in excess of one dollar shall be included in the candidate disclosure statements. Use the Financial Disclosure form with all expenditure receipts attached.

Petition for Candidacy

Petitions for Candidacy are not available online.  Students may only obtain a petition by attending one of the Candidate Information Sessions.  The number of required signatures for each elected office varies. See the Number of Required Petition Signatures for Elected Offices form for information.

Political Organization Candidates List

The Political Organization Candidate List form must be completed and submitted with the Political Organization Registration Form.  This form identifies all of the candidates registering their affiliation with a single political organization. 

Political Organization Registration Form

In order to register as a political organization (also commonly known as a "party" or "ticket"), the organization must register by completing the Political Organization Registration form.  (NOTE:  This form should be submitted in conjunction with the Political Organization Candidates List form.)

Registration Form for Name, Slogan, or Image

All individual student candidates must register any slogans or special images that will be used in campaigning materials with the Student Elections Committee. Political organizations must also register the organization's name, slogan, and special images with the Student Elections Committee.  Complete the Registration Form for Name, Slogan, or Image.

Request for Consideration

Student candidates or political organizations interested in speaking or distributing literature in a classroom, must requested permission from the Student Elections Committee before the activity, by submitting the Request for Consideration form to the Student Elections Committee. Upon receipt, the Student Elections Committee will request authorization and email the student with an approval or decline.

This form can also be used if a candidate or political organization was unable to attend the Candidate Informational Session due to a conflict. The candidate or organization will be required to meet with the Student Elections Committee to review the election process and the information reviewed during the session before any campaigning.

Residence Hall Canvassing Form

A student candidate must complete and submit the Residence Hall Canvassing Form to University Housing Services in order to canvass in any university residence hall. 

Student Elections Grievance Form

Complaints against an individual student candidate or a political organization are adjudicated by the Student Elections Committee.  Any person with a complaint against a candidate or political organization for violation of the Student Elections Code must file a formal grievance using the Student Elections Grievance Form.

Temporary RSO Registration

Temporary RSO status provides candidates and political organizations many of the privileges extended to other registered student organizations (RSOs), such as reduced rates for reserving university facilities. Individual student candidates and political organizations may register with the Student Involvement Center using the Temporary RSO Registration Form.

Withdrawal of Candidacy

Students that register as candidates with the Student Elections Committee may withdraw their candidacies and remove their names from the ballot until the ballot is printed or online by using the Withdrawal of Candidacy form.
