Reporting an Incident

If you are aware of an incident in which the Student Code of Conduct has been broken, complete the Incident Report Form. Once received, Community Rights & Responsibilities will review this report for the purpose of taking appropriate action. Incidents involving criminal behavior will automatically be forwarded to the Illinois State University Police Department for appropriate referral.

Faculty wishing to report acts of academic dishonesty should complete the Faculty Referral for Academic Dishonesty form.
